At Peak Medical Practice, we have set up a new service offering DVLA Group 2 and other occupational medical services in the Peak District, based at Huntbridge Hall, Matlock.
Your D4 form, or other required documentation, will be completed while-you-wait meaning it’s ready to send off immediately, so no waiting for the doctor to get round to completing the paperwork.
We understand a weekday appointment is often not convenient, so offer evening and weekend slots at no extra charge. We can usually perform your medical within 7 days of contacting us.
A standard group 2 DVLA medical costs £50 when performed at our Matlock base, which has free on-site parking and step-free access. Alternatively, we can come to your home or workplace anywhere within Derbyshire for an extra £25. These all-inclusive rates incorporate everything you need to submit your D4 form, including an eye test and examination by a doctor.
Special rates can be negotiated for groups or contract work, and we are happy to consider providing any other medical certification you may need in specific roles.
